Pine Forest School’s EVERGREEN HOURS Before/ AfterCare Registration Form Emergency Form (needed for KinderCare, Before and AfterCare) Providing care for students before and after regular school hours. Staff and Location: A wonderful care staff will serve our Evergreen students this year. The Before and After School programs will be housed in our Big Room. Our KinderCare program will be housed in the Kindergarten Room. Evergreen programs will include indoor and outdoor play, arts & crafts, games & songs, gardening and ground work. Study time will also be available.
Hours: 7:00 a.m. through 6:00 p.m. daily, beginning Monday, August 20th (KinderCare begins Wednesday, August 22nd))
Registration: One Child Two or more Children Before School Only $30.00 $40.00 After School Only $35.00 $65.00 AM and PM $60.00 $115.00 KinderCare (11:30 – 3:10) $75.00
Participation Fees: Before and After School $2.50 per hour KinderCare Only $150.00 per month After 6:00 p.m. $1.00 PER MINUTE – to be added to your bill
Requirements: All participating students MUST be enrolled into the Evergreen Program, with registration fees paid in full and all enrollment paperwork completed by parent or guardian. Parents must sign students in Before School. Parents must sign students out After School. Anyone with an outstanding balance from the 2006-2007 school year will not be allowed to use Evergreen Hours until their account is paid in full. If your account balance becomes delinquent at anytime during the school year, your child will not be allowed to attend the program until payment arrangements have been made. Please make checks payable to Pine Forest School, and note your child’s name/Evergreen Hours in the memo section of your check. Our discipline guidelines mirror those of the regular school day. They are discussed in the student handbook as well as in the Aftercare handout. Children unable to adhere to the discipline guidelines for conduct, may be removed temporarily or permanently from the program. Pine Forest will not provide transportation for the program; there will be no fieldtrips during the program. No medication will be administered by program staff. Parents are always welcome to participate in our program, and are welcome to visit. Please speak with us about scheduling time! If we are unable to reach you during an emergency, your emergency numbers will be contacted. Our staff is first aid & CPR trained. We conscientiously and kindly care for all and sundry bumps, scrapes and other little inevitabilities of good play. Pine Forest Evergreen Hours is regulated by the following: Arizona Department of Health Services Office of Child Care Licensure Flagstaff AZ 86001 (928) 774-2707 Inspection reports available upon request. Our liability insurance is with Willis. The policy is available in the office.
